[摘 要] 目的 探讨对比胰岛素泵和多次胰岛素强化治疗初发2型糖尿病的临床效果.方法 选择2015年1月—2017年5月期间采用胰岛素泵和多次胰岛素强化治疗初发2型糖尿病患者共200例,两组患者均给予糖尿病的相关健康教育、饮食干预、运动干预等措施,不赘述,两组患者治疗区别在于对照组采用的是多次胰岛素强化治疗,观察组采用的是胰岛素泵的治疗.治疗两组后对比两组降糖效果和不良反应. 结果 治疗2周后,两组的空腹血糖、餐后2 h血糖及糖化血红蛋白水平均较治疗前明显降低,差异有统计学意义（P<0.05）,治疗后组间比较,观察组空腹血糖、餐后2 h血糖及糖化血红蛋白水平均低于对照组,差异有统计学意义（P<0.05）, 治疗2周后,观察组患者的总有效率为96%,显著高于对照组82%,差异有统计学意义（P<0.05）,观察组患者出现低血糖反应1例,占1%,对照组患者出现低血糖情况16例,占16%,对照组显著高于对照组,差异有统计学意义（P<0.05）,两组患者均未出现其他并发症.结论 在治疗初发2型糖尿病方面,胰岛素泵和多次胰岛素强化的临床对比,胰岛素泵的降糖效果更佳,不良反应少.

Clinical Comparison of Insulin Pump and Multiple Insulin Enhancement Treatment of Initial Type 2 Diabetes

[Abstract] Objective To study the clinical effect of insulin pump and multiple insulin enhancement treatment of initial type 2 diabetes. Methods 200 cases of patients with initial type 2 diabetes in our hospital from January 2015 to May 2017 were selected, both groups adopted the related health education, dieting intervention and motor intervention, on this basis, the control group adopted the multiple insulin enhancement therapy, while the observation group adopted the insulin pump, and the hypoglycemic effect and adverse reactions were compared between the two groups after treatment. Results After two-week treatment, the fasting blood glucose, postprandial 2h blood glucose, and glycosylated hemoglobin levels obviously decreased compared with those before treatment, and the differences were statistically significant（P<0.05）, after treatment, these indicators in the observation group were lower than those in the control group, and the differences were statistically significant（P<0.05）, after two-week treatment, the total effective rate in the observation group was obviously higher than that in the control group, （96% vs 82%）, and the difference was statistically significant（P<0.05）, and incidence rate of hypoglycemic reaction in the observation group was obviously higher than that in the control group, （1 case vs 16 cases, 1% vs 16%）, and the difference was obvious（P<0.05）, there were other complications. Conclusion The hypoglycemic effect of insulin pump in treatment of initial type 2 diabetes is better than that of multiple insulin enhancement, and the adverse reactions are fewer.
