【摘 要】 目的 探索分析應用强的松龙对重症哮喘急性发作患者实施院前急救的临床作用和效果.方法 96例重症哮喘急性发作患者, 随机分为对照组和研究组, 各48例.对照组常规现场急症救治后予以地塞米松静脉注射, 研究组常规现场急症救治后予以强的松龙静脉注射, 比较两组患者治疗前后呼吸频率、心率、血气指标变化以及不良反应发生情况.结果 治疗后, 两组患者的呼吸频率以及心率均较治疗前显著改善, 且研究组患者的改善情况优于对照组, 差异有统计学意义（P<0.05）.治疗后, 两组患者的相关血气指标均有效改善, 且研究组患者的改善情况优于对照组, 差异有统计学意义（P<0.05）.研究组不良反应发生率为6.25%, 显著低于对照组的29.17%, 差异有统计学意义（P<0.05）.结论 针对重症哮喘急性发作患者应用*强的松龙实施院前急救, 可以有效改善患者呼吸频率、心率以及血气指标, 可以有效控制和减少不良反应发生, 对于提升患者生活质量具有积极意义, 值得推广应用.

【Abstract】 Objective To explore and analyze the clinical effect of methylprednisolone applied in pre-hospital care for acute attack of severe asthma patients. Methods A total of 96 acute attack of severe asthma patients were randomly divided into control group and research group, with 48 cases in each group. The control group received intrenous injection of dexamethasone after regular on-site emergency treatment, and the research group received intrenous injection of methylprednisolone after regular on-site emergency treatment. Comparison were made on breathing rate, heart rate, blood gas index changes before and after treatment and adverse reactions occurrence in two groups. Results Comparing with those before treatment, both groups had significantly improved breathing rate and heart rate after treatment, and the research group had better improvement situation than the control group. Their difference had statistical significance （P<0.05）. After treatment, both groups had improved blood gas indicators, and the research group had better improvement than the control group. Their difference had statistical significance （P<0.05）. The research group had significantly lower incidence of adverse reactions as 6.25% than 29.17% in the control group, and the difference had statistical significance （P<0.05）. Conclusion Application of methylprednisolone in pre-hospital care for acute attack of severe asthma patients can effectively improve breathing rate, heart rate, blood gas index situation, control and reduce occurrence of adverse reactions, and it also shows positive significance in improving quality of life in patients. So it is worthy of promotion and application.
