[摘 要] 目的 研究颈动脉硬化斑块稳定性采取超声检查评价的作用.方法 按照随机抽样方式将该院2016年6月—2017年4月门诊和住院的40例颈动脉硬化斑块患者作为研究对象,均进行超声检查评价斑块稳定性情况,回顾性分析他们的临床资料,观察动脉壁结构和斑块形成的位置,测量斑块大小,观察斑块的内部回声、斑块表面有无溃疡、有无纤维帽及钙化,彩色多普勒显示动脉管腔内的血流情况,以及观察斑块内部及表面血流情况,以此判断斑块性质,总结斑块情况. 结果 40例颈动脉硬化斑块患者,斑块基本为多发性,对较大典型斑块进行测量后显示最小斑块大小0.20 cm×0.15 cm、最大斑块大小2.68 cm×0.37 cm；40例动脉硬化斑块患者中有7例评价为易损斑块,7例易损斑块的回声为不均匀性回声或低回声,斑块纤维帽局部或数处存有缺失,共计6例,斑块表面有溃疡2例、斑块内部有血流3例.结论 彩色多普勒超声检查评价颈动脉硬化斑块稳定性有不错的价值,可测定斑块形态和部位及动脉壁结构,而且可以观察斑块有无纤维帽、斑块表面有无溃疡、斑块内部及表面血流等情况来评估斑块稳定性和否,且可无创性评价稳定性,值得借鉴.

[Abstract] Objective This paper tries to study the effect of color doppler ultrasonography on the stability of carotid atherosclerotic plaque. Methods 40 cases of carotid artery atherosclerosis patients in this hospital from June 2016 to April 2017 were selected as the research objects according to the random sampling method, all the patients were examined by ultrasonography for stability evaluation, and their clinical data were retrospectively analyzed, the arterial wall structure and plaque morphology and position, measurement of plaque size were observed so as to determine the nature of the plaque and summary the situation of plaque. Results 40 cases were of multiple carotid artery atherosclerosis, measurement of the large plaque showed that typical minimum patch of 0.20 cm×0.15 cm, the largest patch of 2.68 cm×0.37 cm；among the 40 cases of atherosclerotic plaques, 7 cases of vulnerable plaque with inhomogeneous echo and low echo, or several local fibrous cap were missing with a total of 6 cases, 2 cases of surface ulcers, 3 cases of blood flow inside the plaque. Conclusion Color doppler ultrasonography in the evaluation of the stability of carotid atherosclerotic plaque is of great value, which can determine the plaque morphology and site and arterial wall structure, and the plaque fiber cap, plaque surface ulcers, plaques internal and surface blood flow and other conditions can be observed to assess plaque stability or not, and can complete non-invasive evaluation of stability, so it is worth reference.
